Chronic Disease and GP Managed Care Plans

We work with you to create a plan which organises and addresses all your health needs over the next 12 months.

This includes planning for bloods, procedures, referrals, and radiology that may be done.

At Tintenbar Medical Centre we have a strong focus on preventative care in chronic disease management, including addressing risk factors such as obesity, smoking and alcohol. We are proactive in working with patients by providing education on how best to manage chronic disease.

Additionally, patients can access many allied health services including podiatry, dietitian, psychology and physiotherapy all from our one convenient location. By having these ancillary services situated together we also ensure a more streamlined and collaborative approach to patient care.

We also have on site pathology and work closely with nearby chemists, making the entire process timely and smooth for our patients.

Comprehensive and individualised care plan can ensure that all aspects of chronic disease are managed including goals to achieve optimal health. Chronic conditions including diabetes, hypertension, osteoporosis, arthritis, chronic pain are amongst the most common conditions that can be supported with the help of a care plan. Some care plan patients are eligible for referral to allied health providers at a subsidised cost.
